New Grants for CDSM Programs

Please know that MA Department of Public Health is pleased to announce its third round of funding for Councils on Aging — Arthritis Integration Dissemination — to deliver My Life, My Health, Chronic Disease Self-Management programs.
    Areas of focus are based on regions of the state with the highest incidence of arthritis and include Franklin and Worcester Counties and southeastern Massachusetts.   We look forward to continuing this exciting partnership with Councils on Aging to build capacity in your communities to empower people to better manage their chronic conditions.   We appreciate your assistance disseminating the request to the COAs.
    You may wish to first review the "Request for Partner Applications" for information regarding the administrative aspects of the program.   This program is well-worth considering:   its applicability to improved health and cost savings to participants is well documented.

Chronic Disease Self-Management Program — Leader Training
The Chronic Disease Self-Management Program and will be offering a promotional question and answer session on Wednesday 4 August 2010 at 1 p.m. for an upcoming 4-day Leader Training.
The actual training will be in September on the following Tuesdays and Wednesdays, 9/7, 9/8;   and 9/14, 9/15.
