Chronic Disease Self-Management Coalition

Attached you will find the 2013 Healthy Living Programs Leader Training Schedule.
We are very excited to reveal the many trainings being offered in all geographical areas throughout Massachusetts! You will notice there is a contact person associated with each training. Please contact that individual directly if you would need more information or would like to register for one of the trainings.
Also attached is the Chronic Disease Self-Management Master Training Application for (2) Master Trainings that will take place in Massachusetts and the Memorandum of Understanding (MOU): State CDSMP Licensure.
Please review and complete the Master Training Application and MOU (if not already submitted) and return to me no later than the dates posted.
